Case Management coordinates needed services to help adults with serious and persistent mental illness in gaining access to needed services including:

  • Psychiatry – Therapy

  • Medical – Medication Management

  • Housing

  • Employement

  • Benefits (Social Security, Medical Assistance, Medicare Part D, etc.)

  • Budgeting

  • Education

Eligibility standards for Case Management:

  • At least 18 years of age

  • Diagnosed with a serious and persistent mental health illness

  • Qualifies for Medical Assistance
